In order to provide individualized care that meets the wants and needs of our clients, we work from a plan of care developed not only from our nurse’s initial assessment, but also with direction from the client’s physician and input from the client and his or her family. This is a function of our geriatric care management team. Our geriatric care management division specializes in coordinating elder care services. It is our desire is to assist seniors, aging adults and their families with the challenges associated with managing the issues that affect their quality of life. We provide elder care and healthcare counseling, crisis intervention, needs evaluation, care planning, and supervision for elderly adults. We are committed to preserving the well-being and dignity of the elderly and in providing assistance to those seeking quality, continuing care with a personal touch. As Professional Geriatric Care Managers we are here to assist you with your individual elder care needs. We coordinate care-giving services so you can have piece of mind in knowing that you or your aged loved one is receiving quality, personalized care.

- Assist with a residential placement when the time comes
